Telligen offers Diabetes Self-Management Education (DSME) classes in Colorado, Illinois and Iowa to people with Medicare diagnosed with diabetes or prediabetes.
Living with Diabetes or Prediabetes
More than 30 million Americans are living with diabetes. Another 84 million are living with prediabetes – a serious health condition that increases the risk of type 2 diabetes and other chronic diseases. Telligen has partnered with the Centers for Medicare & Medicaid Services (CMS) to bring the Everyone with Diabetes Counts (EDC) initiative to communities we serve. The EDC initiative offers diabetes education programs that teach participants to effectively self-manage their diabetes through nutrition, exercise, self-monitoring, diabetes medications and community resources and support. All at no cost!
Telligen uses two different DSME curricula: the Stanford University Diabetes Self-Management Program (DSMP) model and the Diabetes Education Empowerment Program (DEEP) from the University of Illinois, Chicago (UIC). Both programs cover the same topics. Participants learn:
- Self-management skills, problem-solving techniques and how to create action plans.
- Appropriate exercises to maintain and improve strength and endurance.
- How to count carbohydrates, read labels and plan meals.
- Why it’s important to follow medication regimens and actively manage one’s healthcare.
- How to engage with healthcare providers and partner with one’s healthcare team.
- Techniques to deal with the symptoms of chronic disease, fatigue, pain, depression and stress.

Telligen has partnered with organizations with proven histories in providing Diabetes Self-Management Education (DSME) classes throughout Illinois and proudly supports the collaborative effort to build sustainable DSME opportunities in local communities. Diabetes Self Management Education Classes in Iowa:
Telligen has teamed up with communities throughout Iowa to offer a diabetes education program. The program consists of six classes that are 1 1/2 hours. These classes are free of charge! As an incentive, graduates will receive tools to support the new lifestyle and self-management skills they learned in class. Telligen knows making life changes can be difficult – the goal of these classes is to make small change for big rewards! Follow this link to find classes near you.
Telligen was honored for its DSME program at the Heroes Dinner at the Salvation Army Ray & Joan Kroc Corps Community Center in Chicago. The Kroc Center hosts free DSME classes every Tuesday as part of Telligen’s work with the Everyone with Diabetes Counts program.
