Learning Session 4: Applying and Sharing Successes: The 4M’s Framework of an Age-Friendly Health System Outcomes Congress

The purpose of this 9-week Learning Collaborative is to guide participants through building an Age Friendly Health System (AFHS) with the 4Ms (What Matters, Mentation, Medication and Mobility). This collaborative will intertwine the 4Ms with the focus areas of: Infection Prevention, COVID-19, Opioids & Behavioral Health, Patient Safety & Care Transitions, Chronic Disease Prevention & Self-Management, and Nursing Home Quality.
Participants will learn how to apply 4M strategies and tools and meet with community Geriatrics Workforce Enhancement Program (GWEP) representatives to continue establishing an AFHS at their organization.
