Chronic Care Management: Creating a Healthier Community Through Collaborative Care Coordination

Three in four Americans aged 65 and older have multiple chronic conditions and account for 93% of total Medicare spending. The Chronic Care Management (CCM) service is an underutilized Medicare benefit designed to improve health outcomes and lower health care costs while generating revenue for providers. The CCM service increases access to team-based care and care coordination for patients in rural, underserved areas, ultimately resulting in improved health outcomes and healthier communities. This session will describe successful strategies and provide resources that may be modified when implementing programs at the local community level.
1. Identify strategies that care teams may utilize for reimbursement/sustainability of their services in the Chronic Care Management model of care on conditions such as hypertension or diabetes
2. Illustrate what chronic Care Management services are and how pharmacists and providers can engage in implementation and maintenance of the service increasing access to care
3. Describe resources that are available to use when developing and implementing programs
