Integrating Social Work Consultation and Care Coordination into Primary and Specialty Care: The AIMS Model

Complex health-related needs include underlying social, economic, and environmental factors that influence peoples’ ability to be healthy and can increase their risk of hospital readmissions. Research has highlighted the role of effective interprofessional care teams in improving health outcomes for people with complex needs, yet many health systems and practices do not take advantage of the special training of social workers. At Rush University Medical Center in Chicago, social workers provide the Ambulatory Integration of Medical and Social (AIMS) intervention to primary and specialty care patients. By leveraging social workers’ expertise in patient engagement, behavior change, mental health and community resources, practices are able to offer comprehensive and integrated care to support patients aging in the community and to reduce unnecessary hospitalizations. This Telligen webinar with Matthew Vail, clinical social worker in the Department of Social Work and Community Health at Rush University Medical Center, will provide an overview of the AIMS model, including implementation considerations and sustainability mechanisms under Medicare.
