Living the Care Plan: Finding the Person in Person Centered Care and Meeting CMS Regulations

Join Telligen as we explore processes and practices to create person-centered care plans that align with the Requirements of Participation (ROP). The session’s panel includes Lee Moriarty, BS/CTRS, president of the Illinois Pioneer Coalition and Dionne Brown RN, BSN, director of nursing at Summit Rehabilitation and Care Community.They will share their experiences and discuss how to use the minimum data set and other strategies to capture elder goals, routines and preferences which are critical to crafting a person-centered care plan.
