Post-Acute Care Collaborative: Your Role Today in Safer Care Transitions
Post-acute care transitions play a critical role in the next steps for COVID-19 care and infection prevention. Whether discharged from a hospital, social distancing at home (sheltering-in-place), receiving home care, or returning to a nursing home facility there are still so many unanswered questions about how to care for and support patients and families during this challenging time.
Our Post-Acute Care Collaborative Office Hours will feature COVID-19 and Infection Prevention speakers from across the care continuum including subject matter experts, local stakeholders, and partners from your state. This interactive session will conclude with an opportunity to ask questions.
Our guest speakers, Krista Vanden Brink, RN, BA, Administrator, Winneshiek County Public Health Nursing Service and Andrea Kent, RN, New Choices Home Health Administrator in Bettendorf, will discuss the impact COVID-19 has had on home health agencies and county public health. Topics will include: allocation of PPE, the role of public health, emergency preparedness plan, patient education and reducing anxiety, staff support, education, and encouragement, and how home-based care providers fit into the overall COVID-19 emergency in Iowa.
