Promising Practices to Improve Outcomes for People with Cardiovascular Disease and Diabetes

We are pleased to announce that Teri Hernandez, PhD, RN, Associate Dean for Research and Scholarship in the College of Nursing with a secondary appointment in the Department of Medicine, Division of Endocrinology, Metabolism and Diabetes at the University of Colorado and Katherine Overton, Senior Quality Systems Program Manager from the American Heart Association are the speakers for the next Telligen learning session.

According to the American Heart Association and the American Diabetes Association cardiovascular disease is the leading cause of death for people living with type 2 diabetes. This learning session will highlight Promising Practices to Improve Outcomes for People with Cardiovascular Disease and Diabetes. Our presenters will review tools and programs for supporting the implementation of evidence-based best practices for screening and managing of these chronic conditions.

Following this event, participants will:

  • Apply screening tools that identify individuals at risk for diabetes (DM) and/or cardiovascular disease (CVD)
  • Discuss the importance of lifestyle management for diabetes and cardiovascular control for individuals with DM and CVD
