Event Description:

In recognition of Healthcare Quality Week, we invite you to join an interactive community conversation that recognizes our shared healthcare quality improvement (QI) journey and identifies the strongest needs moving forward as we co-create the 2022 results-driven QI road map.

• You bring local expertise, the eyes and ears representation of your home community’s priorities, and ideas for growing our work together. Whether this is your first time joining or you have been with us in TCCC before, we want to hear from you! In that spirit, the Center for Health and Social Care Integration (CHaSCI) will highlight evidence-based care management practices and quality improvement strategies that support care coordination for individuals with complex medical, social, biopsychosocial and functional needs.
• Our team convenes geographically diverse peers, shares actionable and relevant data and wishes to meet you where you are on your QI journey by remaining continuously immersed in your local priorities.

• Collectively, we are in this together. We have the power to coordinate local, community, state and regional action to co-create a more hopeful future in areas that data indicates are most heavily affecting the quality of life of our respective families, friends and neighbors:
  • Opioid utilization and misuse
  • Patient safety: clostridium difficile infection (CDI), adverse drug events and facility acquired infections
  • Chronic disease: cardiovascular health, diabetes and chronic kidney disease
  • Care Coordination: preventable ED visits, avoidable 30-day readmissions, multi-visits/super-utilization of emergency departments and hospitals
  • COVID-19, influenza and pneumonia
