The National Steering Committee for Patient Safety, a group of 27 organizations convened by the Institute for Healthcare Improvement (IHI), has developed a set of actionable recommendations for advancing patient safety. The recommendations cover four foundational areas—culture and leadership, patient and family engagement, workplace safety and a learning system. The strategies outlined in each category are built on substantial bodies of evidence and lessons learned. Committee members will implement, test and refine these strategies as understanding and evidence evolves over time.
